Step 1:

If interested in any of our programs please give us a call at (312) 624-8750.

Step 2:

Next, a member of the LEEP administration team will give you a call and go over some basic information about  your child and what services you are looking for. We will also set up a time for you to come to our clinic without your child to learn more in depth about our programming, discuss your goals, address your concerns, answer any questions you may have, as well as, take a tour of our facility. Please make sure to bring your insurance cards so we may take a copy and provide them to our Insurance Advisor.

Step 3:

You will receive an email from our Insurance Advisor with specific benefits  explanation and payment options.

Step 4:

If interested in moving forward with one of our programs and/or services we would then schedule a consultation for your child with one of our therapists. The fee for this consultation is $50.00 that can later be applied to your services if you choose to enroll.

Step 5:

After your child’s consultation the therapist will meet with a member of our administration team to review current openings in our programming and/or therapists schedules.

Step 6

A trial day will then be scheduled for your child for any group placements. Following your childs trial day another meeting will be scheduled to determine the best fit for your child.

Step 7:

Once placement has been determined for your child he/she can then be enrolled in our program.

Step 8:

An at home evaluation will be scheduled to determine goals and establish a treatment plan.
